What to Bring and Do For Your FirstAppointment

First appointments are a time for you and your therapist to get to know each other, gather necessary information for insurance claims, and gain insight into the problems you are experiencing and determine a treatment plan.  That is a lot to accomplish in 50 minutes, so the more you can get done ahead of time, the more time you will have to talk about your problems.  That is why we have used a secure site for you to enter preliminary information and set up your appointment (see Setting Up Your First Appointment below).  In addition, there are additional forms (see Additional Forms below) that you will be emailed for you to fill out and sign prior to your appointment.  If you will be using insurance, please bring or email if you are doing Teletherapy, your insurance card with you as well as contact your insurance company to help ensure that you have the benefits you are expecting.  After you complete the initial information on-line, we will be alerted and we will also try to verify insurance coverage prior to your appointment.

Setting Up Your First Appointment

Thank you for your interest in our practice.  Taking the first step towards getting help is often the most difficult one.  If you  are ready to schedule an appointment, click on:

This will bring you to the appointment site.  Once there, click on the “I’m new here.. set appointment” button.  It will take you through some basic information, then after complete to a more detailed biographical form. (may take up to 30 minutes to complete) Please complete all the questions you can answer.  When you are finished, go to the set appointment at the end of the questionnaire to set up your own appointment.
